It's probably an M37 w/ a Doage WC 62/63 rear running gear and an extended M37 bed. It is purely custom by someone and not GI. A link to photos of one such animal were posed on the M37 forum last year.
Price? Whatever you think it's worth. It's a custom truck and tough to put a price on.
