My '84 W350 with Cummins, 5 speed, and 3.54 gears can get near 20 mpg if I keep it under 65 mpg.
The worst it has ever gotten is 13 mpg, buring straight vegetable oil, going 78 mph towing a TJ on a trailer.
My '86 Jeep Grandwagoneer with AMC 360, 727, and 3.54 gears got 9 mpg.
