I once had a brand new, Mopar-supplied power steering pump pulley fly right off it's hub after only a few hours of use. The pulley istelf literally broke in two pieces, leaving the hub still on the pump and the rest of the pulley flying around inside the engine compartment.
The failure was never explained. A used pulley replced in its stead has been spinning fine for over a decade so far.
