Front drive shaft would need to be lengthened, and rear shortened. But rear would not need near as much shortening as if you added any other OD system. And that would give you lots of synchros.
I think the shortcomings of the flat heads are built in. Doesn't matter how you drive it, the design flaws will come through. There was a reason that somebody invented the in-chassis-crank-grinder. And the corollary reason why the i-c-c-g when obsolete.
My guess is that a 20% OD will make the engine last 20% longer. ;)
