What usually happens is that the door pins rust into the hinge piece that mounts to the front door post (few people realize that there is a small "V" grove at the top of this hinge piece to oil the pin). When the pin freezes to the hinge piece the door starts to rotate on the door side part of the hinge. This results in the pins eating out the holes in the door side hinges.

The repair is pretty straight forward. I have done it to all of my doors.

1) Take the door off and clean out hole in the door post side of the hinge. I use a small round wire brush to do this with lots of oil. I also put rust killer down the hole when I get it cleaned out. MAKE SURE THAT A NEW DOOR PIN WILL ROTATE FREELY IN THIS HOLE. This hole usually does not wear much as it has a lot of surface area and the pin has usually stopped rotating in it. If it is slightly worn that is OK as the pin needs to rotate freely in this hole.

2) On the door side of the hinge I take a chisel and make an "X" that marks the original center of the hole for the pin (make sure that it is deep and goes well beyond the edges of the holes). It is important to keep the holes in their original positions. I then weld the holes up then grind the surface smooth. MAKE SURE YOU DO NOT GRIND OFF YOUR "X" CENTERING MARKS. Now re-drill the holes so that the pin shaft fits very tightly (use new pins). Install the door using lots of oil on the pin and in the hole in the door post side of the hinge. Use a hammer to drive the knurled portion of the door pin into the hole at the top of the door side hinge. This will stop the door from rotating in the door side hinge holes. Keep the hinge well oiled or the pin will freeze again and the holes will wear again.
