If the axle twisted 5 times, then it was not a case hardened shaft, as it wouldn't have had the toughness to endure that amount of deformation before failure.
Most aftermarket axles have cut splines instead of rolled splines. Cut splines perform nearly equal to rolled splines in one-time high stress situations, but rolled splines are significantly superior in fatique performance due to the forged microstructure.
Sounds like too much stress for the shaft, but the 5 turns does seem pretty unbelievable!
