Drop the clutch cover and inspect the inside area. If you can no longer adjust the linkage to get free play, then three possible items:
Clutch disk worn to the point it's spinning on the flywheel under load causing metal to metal grinding.
disk cover is shot.
throughout bearing shot.
my guess is the first item especially under load and full throttle condition
