I have a 1991 D350 dually with the 727 that gets good mileage (21-25), and pulls hard. It's limited to 73mph with a 3.07 rear. Overdrive (A518) trucks will have 3.55s
The 6bt is heavy, so the front end will be a concern with these first generation trucks. When you are looking ask about it. I didn't, and had an upper control arm fold at 60mph and it wasn't fun, but it's fixed now.
Something this old will have some issues, but I could listen to a 12 valve all day long!
