Check your fuel tank fill cap. It is vented so you dont pull a vacuum on your tank. If the vent is plugged, eventually you will start to pull a vacuum and the engine will starve for fuel.

When you idle her, little fuel is used and it would take a long time to develop a vacuum and see a problem at idle. But when your driving fuel is used faster and a vacuum will develop much quicker.

The getting to temperature when it happens may have nothing with the actual problem, except that it shows you have driven far enough to develop the vacuum at which time the engine is also up to temp.

Try putting the fill cap on loosely so air can come in the tank and then go for a ride. Get her up to temperature, if all is still well, keep driving. If all is well, you know th vent in the fuel cap is plugged.
