Could be bad bolts, but I'd rather have a design that didn't require perfect performance from them. Design it so it would work with grade 2s, with torque anywhere between "seems snug" and "broke the bolt off", and then specify grade 5s just to be on the safe side, and be done with it. There are a few automotive applications where correct alloys, correct heat treat, and correct torque are vital (inside the engine, mostly), but wheels should not be one of those things.
