The front axle is standard PW, so no mods or spacer at all.
You're correct in describing the dually spacer setup; the spacer is simply "sandwiched" in between the two wheels... the studs pass through it, but do not themselves anchor it.
The shank on the stud only serves to keep the spacer centered in position. The lug nuts that secure the outer wheel/tire also serve to hold the spacer in place. Another words, the spacer just kind of lays in there until you mount the outer tire.
