Dodge truck 12" clutch/hydraulic 360 flywheel?

I measured the od of the clutch on my hydraulic V8/NP435 setup. It is 12". It is out of a W500. The Mopar info lists the 11.5" as the largest clutch. Did I mismeasure the plate?

Also, anyone know if the W500 hydraulic clutch setp was ever factory used with the 360? The last owner said it was original. The flywheel has 3 small ~1/2" holes near the outer ring on the engine facing side (like the single hole used for the cast crank 340). I looked at the manuals for Mopar cars/light trucks - and didn't see anything like my flywheel. The flywheel has no part number than I can find.

I am wondering if the flywheel is a poly 318 flywheel balanced to a 360 by an unknown machine shop or (hopefully)Dodge used old stock up on the larger trucks in the early 1970s and balanced my flywheel to the 360 by driling it in an unusual location (assumes my 3 smaller outer holes equate to the typical 3 inner area 1 & 13/64" holes found on 360 flywheels.)

Thanks for help with this odd-ball Mopar flywheel id.
