1963-68 Canadian Door tag help needed

Guys,

I have attached a picture of a 1962 D100 tag from a Canadian built truck (thanks to George for sharing this image with me). I need anyone with a 1963-68 Canadian built truck to look at this tag and let me know if their tag looks like this or not.

If not, please let me know what your tag looks like, what year your truck is, and if possible send me a picture of the tag.

If your tag does look like the one above please let me know the year of your truck so I can narrow further the era this tag was used.
