... you are hearing here. The early Powerstoke diesel has a mechanical lift pump that puts low pressure fuel into the head galleries. These do fail, I just replaced mine, but they would not make noise unless the engine was running, since they are powered by the engine cam.

Electric pumps on Powerstrokes did not come along until 1998 models.

Could you maybe be hearing a clattering sound from a sick glow relay switch? That would track with your cold vs warm engine observation. The oringinal on the 1995-1996 was a recall item - the one on my 1995 actually melted. While I was out camping. 35 miles from the nearest "real" road. Had to short the glow system to life with a screwdriver.
