I think the 6 speed was introduced in '01 when the high output engine came out. IN previous years you would have the 5 speed which is not necessarily a problem. A friend has a '99 and tows a lot with his CTD and 5 speed. The low and high gear on both is the same, its just the 6 speed has closer gears with one more.
It's been 2-3 years, but if I recall the Jacobs brake cost me about $500-$600, and then I paid the Dodge mechanic another $100 or so to install it.
The reason I got a Jacobs is that it would not void my still existing Cummins-Dodge warranty at the time. After your warranty is up(5year or 100k), you are free to use different models of exhaust brake.
If you do get a Dodge diesel, I would also recommend subscribing to the turbo diesel register magazine and forum. THere is a wealth of good information and product assessments on it. Also there are ads for exhaust brakes.
