If the truck is drivable, I doubt that there is anything major wrong with the t-case. I would be more likely to investigate a broken locking hub or axle as the cause of no 4wd. The NP205 Case is bomb proof, and really simple. But, when the go, it tends to be catastrophic. No more driving around...I guess you could have a shift linkage issue, but that is about the only think that could cause the simptoms you describe.

As for mileage, it is possible but unlikely. In 2004, I bought a 93 with 29,000 miles and a 91 with 46,000 miles!! Both were absolutely flawless. If it really is a 38,000 mile truck, it should be tight and should show no wear. A well cared for 138,000 mile truck is a good find as well.
