sounds to me like problems in the lower end of the trans, like the counter shaft.
Had the same thing happend to me once and all I had left was 4th gear also, it turned out one of the counter shaft bearings went out, and you only have 4th left because it is the only one that is direct drive.
What I did to get it to the nearest town was put the t-case in 4-low and drove it into a town.
And if we are talking about the red/white truck in the picture, the np-205 t-case is NOT very lite at all.
