I do not know if your box needs a rebuilt or not, if there is as much of the adjuster sticking up as you say it may not.

I only know that several people I know spent a lot of time and money rebuilding everything else, only to be shocked at the amount of wear the steering box gear had on it once they got it rebuilt.

You may not be in that camp yet, and adjustment may be enough.

If you know anyone else in your area with a PW I would try and borrow a known set of good tires mounted on rims, and then take your truck for a drive.

This is after you follow Keith's advice about looking for play, and fixing any problems you find.

Tires can make a HUGE difference, way more then many people think, and you may need one or more new ones if they have developed problems over the last 20 years.
