Could be driver abuse, or it could have had poor workmanship at some time in the past. Maybe Half Turn Harry and Cross Thread Fred worked on it last.
I would agree that you should replace all of the bolts, nuts and locks with new. All are still available, as they are interchangable with M-37 parts. Be REAL sure you have all of the shrapnel out of there, its got to be clean clean clean!
