If you were running that engine before, and it was good, then I would do the one piston.
If it is an unknown engine, i would look into it little closer. Pull another piston, check end gap, and also plastigage some bearings.
If it comes out the bottom, go for it. No need to mess with a head gasket,. But I don't think it can come out that way.
