You definately have a problem load/no load. On the idler gear that has the 70 needle rollers, are two thrust washers and spacer washer in their proper place? Is there any wear on the clutch gear and shift fork areas to allow back and forth movement of the gear and is the fork installed correctly (flat surface to rear of TC)? These would be the two areas I would focus on assuming the intermediate gear is adjusted properly.

I put my TC in neutral and tranny in 4th every time I start and warm up my truck. I don't have any gears banging around.
