It sounds like you are doing well accumulating information in order to make good decisions.
The description "permanent damage" is very concerning. The nervous system has a limited capacity to heal, so if after a few months you are not gettting better, then you may need to persue a more aggrssive course, as you suggested, before waiting until it is too late for surgery, etc to help.
