Cheap quality pistons and bad gas can cause that problem. or maybe the piston got damage during rebuild or someone dropped something down carb that weakened it and heat finaly finished it off. I have seen quite a few holes burned in pistons but that was on very high compression engines running low octane, thats why I think maybe it was weakened somehow on your flathead. the compression is so low, low octane gas should burn fine
