I have and do run the high octane with a 10 degree advance on all my motors and have experienced the 30 to 40 hp change and that has improved my milage significantly. My 86 Dodge Van has a 318 with a 450 cfm holley two barrel and 727 transmission. I run nothing but Shell 91 octane in her. The coil is a MSD Blaster 6 high vibration with MSD Distributor, cap, rotor and wires. My plugs are Allied signal titaniums they ran about $6 apiece. I am getting between 25 and 28 miles to the gallon and she has 345,000 miles on her. I pulled the engine in feb this year to rebuild her and my buddy who did most of the work said the crank bearings had less then .001 worth of wear on them (thank you LUCAS OIL PRODUCTS) I have kept very good records on all of this as she was my work truck for years. In short higher octane, hotter spark, advance timing just a touch and see what it does for you. I'll never switch back to low octane be fore all these changes I was doing good when I got 15 mpg.
