I have seen a few of those over the years, as far as I can tell they use body parts from a Town Panel, like the rear fenders, and a shortened DOdge nose.

Check the data tag, located on the drivers side of the firewall (if in the stock location) or somewhere inside the cab area.

I am guessing it is a C-1-B6-108.
