The exhaust system is similar to what you described. The tail pipe exits on the passenger side at about the back egde of the cab. The pipe comes almost to the edge of the running board. The muffler sits almost in the frame rail just in front of the transfer case and the exit pipe sneeks between the frame and the transfer case. There are two metal brackets that attach to the frame to hold the muffler at the inlet and outlet. No rubber mounts here. Who cares about the noise transfer. The outlet pipe is a smaller diameter than the inlet pipe (almost seems that it is smaller to let it get between the frame and transfer case.

The stock set up is not what you would call a well designed system. It is not a free flowing system. The smaller tailpipe causes more back pressure. If I were going to change my system I would take it past the axle as you suggest. I would either go dual exhaust or put a larger pipe in and move the muffler under the bed. If you move it under the bed you will need to add a heat shield to the top of the muffler plus put a heat shield on the bottom of the wood bed floor right above the muffler. I would use a more free flowing muffler.
