Not sure what you mean by "flat fender", but I have found no need for engine heaters on any of my WDX-WM300 model POWER-WAGONS in the 25 or so years that I have been driving them. The temperature can get down to -33 degrees here in the Heartland. All of my trucks stay 6-volt. I even had one "work" truck that I parked one year in November and didn't try to start it again the rest of that year. It was parked outside in the wind with the battery left in it. I went outside one morning when it was 20 something below zero, just to see if it would start up. It was a little stiff, but it only cranked about 5 revolutions before it popped off and idled fine with the choke on. Oil pressure was fine.

I know that does not answer your question about increased engine wear. I can't really comment on that other than I don't see it really having a negative effect.
