I have an '03 1-ton dually Dodge CTD High Output with the 48RE transmission. No problems at all with the drive train so far, but it's only got a little over 16,000 miles. My '87 Dodge CTD truck has the venerable A727 with absolutely no problems in over 140,000 miles. My take on the CTD engines is that the automatics seem to be the better choice for longevity and reliability. Of course, my '49 Power Wagon still has the 4-speed manual.
