The block end is going to be a lot closer to water jacket temperature than to exhaust manifold temperature, so if the studs are just a little loose but still have some cast iron to grab onto, I might give the epoxy a try. There is probably some other compound that will take the heat better. J.B. is just metal-filled epoxy, and there are other better varieties out there. You might ask at a serious industrial supply house. I have had some success using black asbestos furnace cement on manifold surfaces that were badly pitted, but I don't know how it would do on a bolt, especially if exposed to water.

The way I figure, if you can torque down the manifold without the stud popping out, before whatever sticky-pucky you use dries, it'll stand a fair chance of holding up for a long time. Otherwise, yes, it's heli-coil time, or if you don't want to spend any money and you're willing to make a g-job of it, just drill and tap it for the next largest stud and drill out the manifold to fit.
