40 mph is right in the middle- Generally, low speed is out of round tire or bent rim, high speed is balance, and on 50 year old trucks, a loose or worn part is a common cause. How far can you wiggle the steering wheel back and forth? Have somebody else wiggle while you look at all the steering linkage joints. See any that don't move together? Now jack it up, watching the knuckles and wheels , make sure everything move together at the same time. Now check the wheel bearings for looseness- can you wiggle the wheels on thier spindles?

There has been a thread on the MV list this week about 'death wobble'. One guy's problem went away when he shimmed the knuckle bearings tighter, another when he replaced tapered roller knuckle bearings with a bronze cone. Seems you need some preload right at the wheels.
