The Gooseneck hitch should be about 1" in front of the axle centerline. The bed support crossmembers are much too frail to support the Gooseneck hitch. I installed a gooseneck hitch in my '98 Cummins long bed and the ball ended up about 44 inches from the back of the bed floor. The hitch rides on top of the frame and is about 150# held in place with grade 8 hardware.
I installed a Diamond Hitch in my 2004, and it was much easier. I highly recommend this hitch as the ball is easily removed and leaves the bed floor flat.
