This can & does happen on Cummins 6BT & 4BT engines...so much so that Cummins has made the Third Gen trucks with electronics that won't allow the engine to run away.
Installing my 2004 Cummins in the W500 I need to attach a magnetic sensor to the flywheel to trick the engine into reving beyond 2000RPM. The new trucks use a sensor in the rear end to sense the vehicle is moving before it will give the engine full rack.
Many people have installed manual air flappers in their First Gen trucks...like you described.
Remember the Cummins truck that was on Ebay with the clutch blown out the side of the bellhousing?
It's uncommon on a stock truck but when you start playing with injection pumps it's common & as a matter of fact, dangerous to play with without disconnecting the air filter & preparing yourself with a piece of wood to block the air to the turbo.
