For my shoulder anchor I used a big assed piece of steel welded to the side of the door jamb and the rounded part of the cab in an "L" shape, this way the headliner should mount up just fine and the anchor is double reinforced,and i am going to use a big washer NOT rely on the bolt or nut threads to withstand the impact. I have had bad luck with threads and sudden impacts. for the lap belts i should be able to use the factory mounting holes, yes my truck came with lap belts!(at least they look factory!) though i dont have them the belt holders are still bolted to the floor.
