Transfer case lever notorious for vibrating in these trucks. They have a nylon bushing in the linkage that may be worn out but be prepared for a rough noisy ol' truck!
You might do well paying Cummins to have a look. A 6BT will easily go past 500K if it's maintained & oil changed every 3K or so but like anything else...put an idiot in charge & he could wreck it before the first oil change.
I'm sure for $100 Cummins would give you their opinion.
May I ask how much they want for it?
