I don't know if any deaths have ever been documented at an operating commercial plant or not, but all of the deaths documented in that "report" happened at government nuclear labs or test facilities, most of them prior to 1960. I'm willing to guess that far more workers have been killed or injured at Dams, coal plants and steam facilities than will or ever will be injured at Nuclear plants. I know for certain that far more civilians and workers have been harmed or killed by air pollution from conventional fired plants due to asbestos, coal dust in the mines etc.
Hopefully someday, solar, wind and fusion generation will make all of this a moot point.
