Rick--I should mention that the cover or "shift tower" and not the whole Trans is the only thing different in a Cab Foward application. As far as make/model--the Dodge medium duty L-Series would use the 541. You can see a photo of one in Kris Wickstead's sweptline.com under Dodge Truck history. Ford and IH also used the 541.
