Horn bracket

I am installing some of the things that were not attached to my new truck, but which arrived in a big box-o-parts.

I have a 6-volt horn, about a foot long, that has two mounting machine screws. I have an "ELL" bracket, that has two holes one one face that obviously mount atop two head bolts. (Two head bolts have a female thread in their heads, which I guess is where I bolt the bracket down?) The other face also has two smaller holes.

I seem to be missing some other intermediate piece to complete the puzzle. Has anybody got a decent picture or diagram of how it mounts up?
